To access this form via the web, go to www.VA.gov
On the right side banner select Board of Veterans Appeals, from that drop down menu select forms, than enter 26-1880 for the form number.
That should bring up the form number and title than click on the highlighted form and it will bring up the form that you can than print.
The instructions on how to complete the form are included with the form.
Please note for the San Antonio area the form needs to be mailed to the North Carolina offices, and remember when indicating an address for the eligibility certificate to be mailed to make sure it is an address that you will be able to retrieve mail from, even after you leave Germany.
The instructions will also ask you to attach either you active duty statement or your DD 214.
